Equal Opportunity & Title IX


The Office of Equal Opportunity & Title IX receives and reviews all reports of discrimination, harassment and gender-based violence. Such reports might include allegations of:

1. Harassment, discrimination, and bias incidents on the basis of race, color, national origin, age, religion, disability, sex, sexual orientation, gender identity, gender expression, marital status, genetic information, or veteran status.
2. Sexual Assault (including Rape, Sodomy, Sexual Assault with an Object, Fondling, Incest and Statutory Rape)
3. Stalking
4. Domestic Violence and Dating Violence

If you experienced, witnessed or have received a report of discrimination, harassment and gender-based violence please complete the following Incident Report to the best of your ability. A member of our staff will be in contact with you to follow up on your concern shortly. Is this report confidential? No. Please know that this report is not confidential and the Office of Equal Opportunity & Title IX not a confidential resource. This means that, while we will maintain your privacy, there may be some people who have a "need to know" to whom we will share your information. These individuals will be discreet and respect the privacy of all individuals involved in the process. If I am the survivor or person who was subjected to the discrimination, harassment and gender-based violence, can I fill out this form anonymously? A party who has experienced discrimination, harassment and gender-based violence may report anonymously. However, the University may be limited in the amount of follow up that can be provided if there is insufficient information.

If I am a DU employee, may I report anonymously? No. If an employee learns of any allegations of discrimination, harassment and gender-based violence they must report what they learn and include their name in the report. The only exception is if the employee is the survivor or person who was subjected to the discrimination, harassment and gender-based violence.
