Challenge Program Applications for the 2024-2025 School Year 

Hard Copy (Preferred Method)

Hard copies of the application in both Spanish and English will be handed out at the Information Meetings and available at OMMS or through elementary GT teachers for those who can not attend. Hard copies must be submitted to Orchard Mesa Middle School.

If you prefer, an electronic form is available below.  Electronic forms (Parts 1, 2 and 3) must be completed and submitted by the appropriate person before the deadline.  Once submitted, you will receive an email allowing you to edit your submission if needed.
