Dear Patient,
Please complete the following patient survey if you have a scheduled appointment at The Women's Health Center for Gastroenterological Diseases and Disorders (GIWH). The survey must be completed at least 24 hours before your appointment. Do not complete the survey if you do not yet have an appointment.
The entire survey make take up to 45 minutes to fully complete depending on your medical history and degree of symptoms. Do not go back or refresh this page until you have finished and submitted the survey. You do not have the abillity to save & return later so begin the survey only when you are available for the full anticipated time for completion. The first (current) page of this survey asks for your personal information. The second page asks for deidentified information to assess your symptoms, wellness, and nutrition. All of your responses (personal and deidentified information) are securely stored on an HIPAA-compliant electronic database maintained by Stony Brook Medicine.
The survey enables your medical team to assess your needs and direct your care prior to being seen at the GIWH. Therefore, we kindly ask that you fully and accurately answer the survey to the best of your ability. Your full medical history will be taken during your appointment.
