California Advancing and Innovating Medi-Cal (CalAIM) is designed to improve the quality of life and health outcomes of Med-Cal Members by implementing broad delivery system, program, and payment reform across the Medi-Cal program. CalAIM establishes the framework to address social determinants of health and improve health equity statewide. As part of CalAIM, California received targeted expenditure authority for the Providing Access and Transforming Health (PATH) initiative as part of its section 1115 demonstration renewal to scale whole person approaches to care statewide with a clear equity lens, ensure a strong foundation for integrated, comprehensive care, and a smooth transition from Whole Person Care (WPC) and Health Home pilots that retains investments made by the state, local partners, and the federal government. PATH provides tools and resources to county and community-based providers including public hospitals, county, city and other government agencies, justice agencies, community-based organizations (CBOs), Medi-Cal Tribal and Designees of Indian Health Programs and others to ensure a successful implementation of Enhanced Care Management (ECM) and Community Supports.
PATH is comprised of multiple aligned initiatives that will support implementation of ECM and Community Support services in varying ways. The remainder of this document is focused on the PATH-funded Collaborative Planning and Implementation Initiative, described at a high-level below.
The Collaborative Planning and Implementation (CPI) Initiative will provide funding to support regional collaborative planning by Medi-Cal Managed Care Plans (MCPs), providers, CBOs, city and county agencies, other local governmental entities, public hospitals, Medi-Cal Tribal and Designees of Indian Health Programs, and others to support the implementation of ECM and Community Supports. Stakeholders in a region will form CPI collaboratives that will work together to identify, discuss, and resolve CalAIM implementation issues. PATH-funded collaboratives will be identified by region, taking into consideration the county(ies), topics of focus, and existing collaborative groups within a particular region. Each region will be assigned an approved facilitator by DHCS and the PATH Third Party Administrator (TPA) to support the launch and management of the CPI collaborative. A “region”, as defined by DHCS for this initiative, may consist of one or more. Regional or county CPI collaboratives already conducting collaborative planning and implementation efforts may apply to receive additional PATH supportive funding under this initiative and can request the facilitator they are currently using to serve as the PATH facilitator. Each collaborative will have a designated facilitator approved and assigned by the PATH TPA in conjunction with DHCS. Stakeholders in a particular region or county may submit letters of support to select a different facilitator other than the one initially assigned to their collaborative. Funding will be disbursed to the designated PATH facilitator in each collaborative to support its operations in that region. Individual collaborative participants will not receive direct funding as part of this initiative. This registration form is designed to gather information for the TPA to match entities interested participants in participating in CPI with local collaborative and the associated facilitator in their chosen region. Upon receipt of the interested participant's Registration Form, the TPA will review the forms and notify the participant of their assigned PATH CPI collaborative and associated facilitator. CPI Collaborative facilitators will reach out to assigned participants to engage in collaborative planning and implementation efforts including upcoming convenings.

Registration forms will be reviewed on a rolling basis by the TPA for completeness and accuracy. Following approval, the TPA will match participants with their regional CPI Collaborative and associated facilitator.
