PLEASE READ: This form is used to request a schedule change for INCORRECT or REPEATED courses ONLY! You can submit this form ONE TIME only. Make sure you have reviewed your entire schedule. DEADLINE to submit a schedule change form is SEPTEMBER 15, 2023. After that date, your schedule CANNOT be changed. Your School Counselor will email you if your request was approved or declined. Please allow 24-48 hours to receive a response.
