Before students can begin checking out book from the Elysian Heights Arts Magnet Library, please read the protocol below, then enter the requested information.  Completing this form will serve as parent authorization for their child to check out books.  Please submit separate forms if you have more than one student at EHAM. Please note that all outstanding books and fines must be cleared before new books can be checked out.
