Meal Order Form for Clark Students
Martin County School District - Stuart, FL

Food Service will be providing meals for students who sign up each Monday. To place an order please complete this form. Menu options will either be a hot or cold entrée.

Special Note: The MCSD Food & Nutrition Services (FNS) Department is experiencing unprecedented food cuts without prior notice from our suppliers. These shortages and modifications are causing significant menu changes. We feel it is important to share this information with our students and families. We are working closely with our distributors to secure supplies needed to support our program and offer the level of service you have come to expect. For menu information, please contact Chad Samuels, Cafeteria Manager via phone at 772-221-7125 ext. 48150 or via email at samuelc@martinschools.org.
