ASB Request Form

Thank you for your consideration and interest in ASB support. Please fill out this form to request ASB support services for your event, club, program, etc.

ASB services include the following:

- Posters (3' x 2')

- Flyers (8.5" x 11")

- Lancer Lately segments (not to exceed 30 seconds)

- Green/white/black balloon clusters (no arches)

- Audio/Visual needs (Projection Screen, sound, music, with the exception of the MPR)

ASB services DO NOT include:

-MPR audio/visual. The MPR is equipped with its own sound and projector system. If you require the use of this, please connect with Jody Benon, Activities Secretary, in order to reserve the yellow sound bag.

As the requesting party, it is your responsibility to set up this sound bag with the included instructions.

This form must be submitted at least TWO (2) WEEKS prior to your event. Any requests not submitted two (2) weeks prior will not be considered due to scheduling and time allotments for request fulfillment. Thank you for your understanding.

The ASB Advisor (Mr. Smith) and ASB Executive Cabinet receive and act on this Request Form.

Once this ASB request form is submitted, I understand the following:

1. The requesting group will receive a confirmation email from the ASB Advisor within 48 hours of our request

2. It is the responsibility of the requesting group to confirm all requests with the ASB Advisor at csmith@conejousd.org THREE (3) DAYS prior to the event date.
