Thank you for your interest in Eye Thrive's Mobile Vision Clinic at Deaconess Center for Child Well-Being! Appointment slots are limited, and will be available on a first-come, first-serve basis. Children and youth ages 4 years-old to 18-years-old are eligible.
After completing this form, you must also register on-site beginning at 8:45 AM on July 21st.
A consent form must be completed by the child's/youth’s parent or guardian. The form may be downloaded in English (

click here) to print and complete at home or completed and submitted at the registration table on the day of the event.
All services provided are free of charge, and most glasses will be made and dispensed on-site following the exam.
Anyone over the age of 2 must wear a mask when registering, checking in, and while on the Mobile Vision Clinic. Only patients are allowed on the Mobile Vision Clinic - due to limited space, parents and guardians are not able to accompany patients on board.
Deaconess’ Summer Meals Program will be available from 7:30 AM - 9:30 AM. To-go breakfasts and lunches will be available for children and youth 18 and under. Meal recipients do not need to be present.
