2023 Charleston School District
After School Program
Sports Physicals (for current 6th-11th grade) will be completed at the school based health
clinic on May 7, 2024 & May 14, 2024. If you plan to participate in any sports, this includes
marching band for high school only per the new triple a rules, please return the completed
2 sided pre-participation physical evaluation form to the school nurse by April 30,2024.
Forms will be going home with students. (click here for form)
